I,                   , hereby affirm that I am a certified diver and cognizant of all of the inherent danger of skin and scuba diving, and of the basic safety rules for underwater activities.

I desire to dive with the Nervous Wrecks Dive Club and I understand and agree the purpose of our diving activities is strictly recreational, and that it is not the function of the Nervous Wrecks Dive Club to serve as the guardian (s) of my safety.

BY SIGNING THIS DOCUMENT I ASSUME ALL RESPONSIBILITY FOR PERSONAL INJURY, PROPERTY DAMAGE OR WRONGFUL DEATH, EVEN IF CAUSED BY THE NEGLIGENCE OF OTHERS.

I understand and agree I am participating  in these recreational diving activities voluntarily, and  I agree to be completely responsible for my own actions.

I hereby declare I am of lawful age and legally competent to sign this Agreement, that I understand the terms herein are contractual and not a mere recital, and that I have signed this document of my own free will.

I HAVE FULLY INFORMED MYSELF OF THE CONTENTS OF THIS AGREEMENT BY READING IT BEFORE SIGNING IT & AGREE TO ABIDE BY THE RULES LISTED.

THIS EVENT WAS CREATED BY THE CAPE BRETON NERVOUS WRECKS DIVE CLUB HAS TO DATE HAS RAISED OVER $45,000. DOLLARS AND GIVEN OVER 1200 PRIZES. THE SUCCESS OF THE EVENT IS DEPENDANT ON THE MANY VOLUNTEERS IN THE CLUB, THE GENEROSITY OF THE PRIZE SPONSORS AND YOUR SUPPORT AND PARTICIPATION.

RULES & REGULATIONS
1. ALL DIVERS MUST PRESENT THEIR C-CARD
2. ALL DIVERS MUST WEAR A B.C. (EVEN WITH A DRYSUIT)
3. MIN. EQUIPMENT REG., OCTO, SPG, DEPTH & TIMER.
4. BUDDY PROCEDURES WILL BE FOLLOWED.
5. DIVERS UNDER 16 MUST BE ACCOMPANIED BY AN ADULT.
6. ONLY RETRIEVE 1 TOKEN.
7. REPORT TO MARSHALLING TABLE IMMEDIATELY UPON EXIT WITH YOUR BUDDY.
8. THE DECISION OF THE DIVE MASTER IN ALL DIVE RELATED MATTERS WILL BE FINAL AND NOT OPEN TO DEBATE.
9. NON-COMPLIANCE WITH ANY OF THESE REQUIREMENTS WILL RESULT IN DISQUALIFICATION FOR PRIZES OR REFUSAL OF PARTICIPATION IN GO FOR GOLD. THESE RULES ARE FOR THE SAFETY OF ALL PARTICIPANTS.
